Direct Deposit / ACH Authorization

I hereby authorize Her Alibi LLC, hereinafter called COMPANY, to initiate credit entries to my account indicated below at the depository financial institution named below, hereafter called DEPOSITORY, and to debit the same to such account. I acknowledge that the originations of ACH transactions to my account must comply with the provisions of US law.

A copy of a voided check may be required.

This authorization is to remain in full force and effect until COMPANY has received written notification from me of its termination in such time and in such a manner as to afford COMPANY and DEPOSITORY a reasonable opportunity to act on it.

Key Control Policy

Purpose

The purpose of the Key Control Policy is to protect the property and privacy of Her Alibi Salon, and of individuals assigned to use the facilities, by limiting access to such facilities to assigned individuals.

Scope

This policy applies to all Her Alibi Salon employees. It applies to all facilities owned, leased, or otherwise occupied by Her Alibi Salon.

Issuing Keys

Keys to Her Alibi Salon will be issued to employees with the approval of the salon owner or supervisor as deemed necessary. Each key issued shall be documented. No key shall be transferred from one person to another without being returned to the owner or supervisor for appropriate re-issue.

Building Access

Employees issued facility keys understand access is based on an as-needed basis. Additional personnel such as family members, friends, or other employees are not to be admitted into the facility during non-business hours without the expressed written consent of the Her Alibi Salon owner.

Duplication

All keys referred to in this policy are the property of Her Alibi Salon and are not to be duplicated by anyone. Duplication of a key, or possession of an unauthorized duplicate, may result in appropriate disciplinary action.

Key Retrieval

Upon termination of employment, all keys must be returned immediately to the owner or supervisor. Failure to return keys upon termination or separation may result in holding the employee responsible for the cost to re-key the office or building. Failure to return issued keys will result in a minimum fee of $100 per key.

Lost or Stolen Keys

Lost or stolen keys must be reported to the owner or supervisor within 24 hours of discovery. A replacement key may be issued at cost to the individual. Considering the circumstances, the owner will decide whether to replace the key or require a complete core change. All costs incurred, including labor and materials for re-keying, shall be assumed by the employee who lost the key.

Employee Agreement

My signature below confirms that I have read the Key Control Policy and understand that I am responsible for following it while employed with Her Alibi Salon.
